Instructions for filling out the accident insurance claim form.

This is the insurance form that needs to be filled out for injured players. Do the best you can with the information in Part 1 and then get these to Marian Phillips via the concession stand. She will then complete the league sections and send off to Williamsport. If necessary include enough supplemental information on a separate piece of paper to help Marian fill in Part 2 of the form.   The Little League insurance is secondary coverage but it does kick in to supplement the player's primary coverage. However, the form must be filled out and filed with Williamsport.

This volunteer application is REQUIRED each year from ALL persons who wish to volunteer their services to North Kitsap Little Leagues. All areas must be filled in completely. We take the protection of the youth of our community seriously and the background screening is one of the tools we use to help keep them safe.Thank you for your support.
